Payment and Cancellation Policy

We have chosen autonomy with the strong belief that it leads to better results, more effective engagement between patient and practitioner, and the freedom to move forward without uninvested parties limiting those options.

This has the following implications:

    • We do not accept ANY insurance - payment is made by credit or debit card (HSA/FSA cards accepted), cash, or check
    • Payment in full isĀ expected at the time of your visit
    • At the end of your visit, you will be provided with a receipt detailing the services performed, the amount paid for the visit, and the manner in which it was paid. You may also receive a Superbill, which is required by all insurers and includes all the usual and necessary medical codes.

    • Mindful Medicine Rochester accepts no payments from insurance and we do not file any insurance claims.Ā However, if yourĀ insurance plan allows forĀ out-of-network benefits, we will provide you withĀ a Superbill with diagnosis andĀ procedural codesĀ appropriate to your visit. These approximate the standard reimbursements for the usualĀ "CPT" codes recognized by payers. You may submit this Superbill to your insurance for reimbursement. This does not apply to Medicare (or Advantage plans), Medicaid, or Disability Insurance. If you have one of these plansĀ you will not receive a Superbill.

Cancellation Policy:

    • Your appointment is reserved especially for you.Ā  We understand that sometimes schedule adjustments are necessary.
    • Please understand that when you forget, cancel, or change your appointment without giving enough notice, we miss the opportunity to fill that appointment time, and other clients miss the opportunity to receive services.
    • All cancellations and appointment changes must be made no less than 24 hours prior to the appointment time.

Cancellations made less than 24 hours prior to the appointment time will be charged a $75 cancellation fee that must be paid before another appointment will be scheduled.
